末端瓣膜式耐高压注射型PICC在神经外科围手术期的应用

　　[摘要]目的探末端瓣膜式耐高压注射型经外周中心静脉导管（PICC）在神经外科围手术期患者中的应用效果。方法选取深圳市人民医院神经外科普通病房及NICU病房2019年11月至2020年4月收治的80例行末端瓣膜式耐高压注射型PICC的手术患者为观察组，选取同期行外周静脉导管（PVC）联合中心静脉导管（CVC）禾（或）三向瓣膜式PICC的80例手术患者为对照组。比较两组置管时间、置管维护时间及费用、静脉治疗并发症发生情况（包括静脉炎、导管堵塞、药物外渗、导管相关性血流感染、非计划脱管）、患者满意度（包括操作技术、生活护理、健康教育、服务态度、整体印象）。结果观察组患者置管时间、置管维护时间短于对照组，费用高于对照组，差异有统计学意义（P<0.05）;观察组患者导管堵塞及并发症总发生率均低于对照组，差异有统计学意义（P<0.05）;观察组患者对护理人员操作技术的满意度高于对照组，差异有统计学意义（P<0.05）。结论在神经外科围手术期患者中应用末端瓣膜式耐高压注射型PICC会增加患者费用，但有利于降低置管时间和维护时间，降低并发症和导管阻塞率，提高患者对操作技术的满意度。

　　Application of power injectable PICC in the perioperative period of neurosurgery

　　[Abstract]Objective To investigate the effect of power injectable peripherally inserted central catheter （PICC）in the perioperative period of patients with neurosurgery. Methods A total of 80 surgical patients undergoing power injectable PICC admitted to the general ward and intensive care unit （ICU）of the Department of Neurosurgery in Shenzhen People's Hospital from November 2019 to April 2020 were selected as the observation group. Another 80 surgical patients who underwent peripheral venous catheter （PVC）combined with central venous catheter （CVC）and（or）three-way valve type PICC in the same period were selected as the control group. The catheter indwelling time，catheter maintenance time and cost，occurrence of complications of intravenous therapy （including phlebitis，catheter blockage，drug extravasation，catheter-associated bloodstream infection and unplanned extubation），and patient satisfaction （including operation technique，life care，health education，service attitude and overall impression）were compared between the two groups. Results The catheter indwelling time and catheter maintenance time in the observation group was shorter than that in the control group，but the cost was higher than that in the control group，with statistically significant differences （P<0.05）. The incidence of catheter blockage and total complications in the observation group was lower than that in the control group，with statistically significant differences （P<0.05）. The satisfaction of patients with the operation techniques of nursing staff was higher in the observation group than in the control group，with statistically significant differences （P<0.05）. Conclusion The application of power injectable PICC in the perioperative period of patients with neurosurgery will increase the cost for patients，but it is conducive to shortening the time of catheter indwelling and maintenance，lowering the incidence of complications and catheter blockage，and improving the satisfaction of patients with the operation technique.
