iRoot BP Plus用于乳磨牙活髓切断术的临床疗效

　　摘要：探讨iRoot BP Plus在乳磨牙活髓切断术患者中的临床效果。对照组采用自固化磷酸钙糊剂治疗，观察组采用iRoot BP Plus治疗，术后分别于3、6、12个月评价患者效果，比较2组成功率、牙齿有无松动、叩痛、窦道、修复体是否完整、盖髓剂下方有无牙本质桥形成，根尖有无病变、视觉模拟疼痛（VAS）、牙龈健康状况。结果表明：观察组与对照组术后3、6个月成功率无统计意义（P>0.05）;观察组术后12个月成功率高于对照组（P<0.05）;观察组术后6、12个月叩痛、窦道、修复体不完整、盖髓剂下方有牙本质桥形成、根尖有病变发生率低于对照组（P<0.05）;2组术后3个月GI、SBI指数无统计意义（P>0.05）;观察组术后3个月VAS评分低于对照组（P<0.05）;术后6、12个月GI、SBI指数低于对照组（P<0.05）。

　　Abstract：To investigate the clinical effect of iRoot BP Plus in patients undergoing pulpotomy for deciduous molars， the control group was treated with self-curing calcium phosphate paste， and the observation group was treated with iRoot BP Plus. The patients effects were evaluated at 3， 6 and 12 months after the operation. The power of the two components， whether the teeth were loose， percussion pain， sinus， restoration， whether it is intact， whether there is a dentin bridge under the pulp capping agent， whether there is any disease in the root tip， visual analog pain （VAS）， and gum health were compared. Results showed that the success rate of observation group and control group was not statistically significant at 3 and 6 months after operation （P>0.05）; the success rate of observation group was higher than that of control group at 12 months after operation （P<0.05）; the incidence of percussion pain， sinus tract， incomplete restoration， formation of dentin bridge under pulp capping agent， and apical lesions at 6 and 12 months was lower than that of the control group （P<0.05）; the GI and SBI indexes of the two groups at 3 months after the operation was not statistically significant （P>0.05）; the VAS score of the observation group was lower than that of the control group at 3 months after the operation （P<0.05）; the GI and SBI indexes were lower than the control group at 6 and 12 months after the operation （P<0.05）.

　　牙髓切断术属于一种较为常用的牙髓保存的治疗方式，目的是去除感染的冠髓，保留正常的根髓组织，并在根髓断面放置髓剂并进行严密的冠部充填[1]。因此，患者手术过程中选择何种盖髓剂对改善患者预后具有重要的意义。自固化磷酸钙糊┦侨槟パ阑钏枨卸鲜踔谐Ｓ玫母撬杓粒具有良好的生物相容性，其成分与人体骨组织、牙齿的主要成分相似，可与宿主的骨和牙齿组织紧密结合，能提供适合新骨沉淀的生理基础，具有骨诱导性[2]。但是，自固化磷酸钙糊剂临床使用时固化时间较长，且使用时需要手工配制，导致使用后牙齿容易变色，临床使用受到限制。而iRoot BP Plus是一种新型的生物陶瓷材料，其主要成分为硅酸钙、硫酸钙、氧化锆、磷酸钙等，具有良好的理化和生物学性能[3]。同时，iRoot BP Plus具有固化时间段，牙齿不变色等优点，能做到即取即用，操作亦相对简单[4]。因此，本研究以承德市口腔医院儿童口腔科就诊的患儿中乳磨牙深龋近髓或早期牙髓炎患者为对象，探讨iRoot BP Plus在乳磨牙活髓切断术患者中的临床效果。
