王爱华教授基于“敦土利水”理论治疗克罗恩病经验

　　 〔摘要〕 克罗恩病属于炎性肠病的一种类型，发病后迁延y愈，并发症较多，缺乏特异性治疗方法。中医药治疗肠道炎性疾病历史悠久。王爱华教授基于“敦土利水”理论，认为克罗恩病中医病机以“脾肾阳虚”为本，“湿、热、气滞、血瘀”为标，标本互结致“脾肾虚，湿热滞”，脾肾两虚贯穿克罗恩病发生发展的全过程。王爱华教授总结并应用古籍相关理论，针对以腹泻为主要临床表现的克罗恩病，结合临证经验，提出健脾温肾以治本，结合清热利湿、活血化瘀、固涩止泻以治标，强调固本与驱邪同步，标本兼顾，缓解患者临床症状，促进内镜下黏膜愈合，并结合医案一则，以期指导腹泻型克罗恩病不同阶段的中医辨证用药。

　　Professor WANG Aihua's experience in treating Crohn's disease based on the

　　〔Abstract〕 Crohn's disease is a type of inflammatory bowel disease， which is difficult to cure after onset， with many complications and lack of specific treatment. Traditional Chinese medicine has a long history of treating intestinal inflammatory diseases. Based on the theory of "building up earth to promote flow of water"， professor WANG Aihua believes that the pathogenesis of Crohn's disease in traditional Chinese medicine is based on "deficiency of spleen and kidney"， and "dampness， heat and Qi stagnation" as the treetop. The root and treetop are intertwined to cause "deficiency of spleen and kidney， dampness and heat stagnation"， and the deficiency of spleen and kidney runs through the whole process of occurrence and development of Crohn's disease. Professor WANG summarized and applied the relevant theories of ancient books， aiming at Crohn's disease with diarrhea as the main clinical manifestation， and combined with clinical experience， proposed strengthening the spleen and warming the kidney to treat the root， treat the treetop by clearing away heat and dampness， promoting blood circulation and removing stasis， and fixing astringency and stopping diarrhea. He emphasizes that the root should be fixed and exorcising pathogens should be synchronized， and the root and treetop should be treated simultaneously， so as to alleviate the clinical symptoms of patients and promote the healing of mucosa under endoscopy， and combined with a medical case， the aim is to guide the traditional Chinese medicine syndrome differentiation and medication in different stages of diarrhoeal Crohn's disease.
