盆腔血管3D可视化在妇科恶性肿瘤精准切除术中的作用

   【摘要】 目的:探盆腔血管3D可视化在妇科恶性肿瘤精准切除术中的作用。方法:选取江西省萍乡市妇幼保健院2019年3月-2021年3月收治的60例妇科恶性肿瘤患者,将其随机分为试验组(n=30)与对照组(n=30)。试验组采用盆腔血管3D可视化辅助肿瘤精准切除术,对照组采用常规腹腔镜术。观察两组术中出血量、术中热缺血时间、手术时间、术中转开放率及住院天数,评价两组生活质量(Karnofsky performance status,KPS),分析两组去甲肾上腺素(norepinephrine,NE)、皮质醇(cortisol,Cor)、P物质(substance P,SP)水平,并统计两组并发症发生情况。结果:试验组术中出血量少于对照组,术中热缺血时间、手术时间及住院天数均短于对照组,差异均有统计学意义(P<0.05)。试验组术中转开放率低于对照组,差异有统计学意义(P<0.05)。试验组治疗后3个月的KPS评分高于对照组,差异有统计学意义(P<0.05)。试验组治疗后3个月的NE、Cor、SP水平均低于对照组,差异均有统计学意义(P<0.05)。试验组治疗后的并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:盆腔血管3D可视化有助于提高妇科恶性肿瘤患者的手术效果与生活质量,改善其应激状况,保证其治疗安全性。

   The Role of 3D Visualization of Pelvic Blood Vessels in Precise Resection of Gynecological Malignant Tumors/PENG Wei. //Medical Innovation of China, 2021, 18(36): 0-017
   [Abstract] Objective: To explore the role of 3D visualization of pelvic blood vessels in precise resection of gynecological malignant tumors. Method: A total of 60 patients with gynecological malignant tumor admitted to Pingxiang Maternal and Child Health Hospital of Jiangxi Province from March 2019 to March 2021 were selected, they were randomly divided into experimental group (n=30) and control group (n=30). The experimental group was treated with pelvic blood vessel 3D visualization assisted precise tumor resection, and the control group was treated with conventional laparoscopy. The intraoperative blood loss, intraoperative warm ischemia time, operation time, intraoperative conversion rate and hospitalization days of the two groups were observed, the quality of life (Karnofsky performance status, KPS) of the two groups was evaluated, the norepinephrine (NE), cortisol (Cor), substance P (SP) levels were analyzed, and the complications were counted. Result: The amount of intraoperative blood loss in experimental group was less than that in control group, and the intraoperative warm ischemia time, operation time and hospitalization days were shorter than those in control group, the differences were statistically significant (P<0.05). The intraoperative conversion rate of the experimental group was lower than that of the control group, the difference was statistically significant (P<0.05). The KPS score of the experimental group 3 months after treatment was higher than that of the control group, the difference was statistically significant (P<0.05). NE, Cor and SP levels in experimental group were lower than those in control group 3 months after treatment, the differences were statistically significant (P<0.05). The incidence of complications after treatment in experimental group was lower than that in control group, the difference was statistically significant (P<0.05). Conclusion: 3D visualization of pelvic blood vessels can help improve the effect of surgery and quality of life of patients with gynecological malignant tumors, improve their stress status, and ensure the safety of their treatment.
