基于数据挖掘分析黄明河主任治疗慢性萎缩性胃炎经验

　　【摘 要】 目的：分析S明河主任治疗慢性萎缩性胃炎病例的组方用药规律，为临床诊疗慢性萎缩性胃炎提供新思路与方法。方法：对深圳市中医院2019年1月至2020年6月黄明河主任脾胃科专家门诊治疗慢性萎缩性胃炎的病例中的用药基本信息进行统计并导出;结合K均值聚类、关联规则等数据挖掘方法分析其用药规律，并对结果进行网络可视化展示。结果：共纳入病例342份，涉及中药165味;其中补虚药使用频次最高，出现频次最高的核心药物组合分别为：“党参-麸炒白术”“党参-黄芪”“党参-醋莪术”，核心类方为萎胃汤。结论：黄明河主任认为慢性萎缩性胃炎的基本病机为脾胃虚弱、运化失常;气滞血瘀为其发病的基本病理过程;瘀毒互结是重症CAG及癌前病变的最终结局。治疗上应以益气健脾、理气化瘀为主，佐以解毒散结，临床疗效颇佳。

　　Abstract：Objective To analyze the prescription and medication rules of Director Huang Minghe for the treatment of chronic atrophic gastritis， and to provide new ideas and methods for clinical diagnosis and treatment of chronic atrophic gastritis.Methods Statistics and derives the basic information of medications in cases of chronic atrophic gastritis in the outpatient treatment of chronic atrophic gastritis by Huang Minghe， director of the Department of Spleen and Gastroenterology， Shenzhen Hospital of Traditional Chinese Medicine from January 2019 to June 2020; combined with K-means clustering and association rules and visualize the results on the network.Results A total of 342 cases were included， involving 165 traditional Chinese medicines. Among them， tonic drugs were used most frequently， and the core drug combinations with the highest frequency were： “Dangshen， Baizhu”， “Dangshen， Huangqi”， “Dangshen， Curcuma Vinegar” ，the core prescription is Weiwei Decoction.Conclusion Director Huang Minghe believes that the basic pathogenesis of chronic atrophic gastritis is weakness of the spleen and stomach， and abnormal movement and transformation; stagnation of qi and blood stasis is the basic pathological process of its onset; mutual accumulation of blood stasis and poison is the final outcome of severe CAG and precancerous lesions.The treatment should be based on replenishing qi to invigorate the spleen， regulating qi and dissipating blood stasis， along with detoxification and dissipating masses， with good clinical effects.
