基于数据挖掘的《中华医典》治疗银屑病内服方药规律研究

摘 要 目的：基于中医传承辅助平台V2.5软件分析《中华医典》中治疗银屑病的内服方的组方用药规律。方法：收集《中华医典》收录古籍中所载的治疗银屑病的内服方，利用中医传承辅助平台建立数据库，分析组方的用药特点及规律。结果：筛选出方剂共33条，其中共涉及107味中药，药性多为苦、甘、辛，多归肝、脾、胃经，分析得出用药以防风、大黄、当归为主。支持度≥6的中药药对组合为9对；分析熵层次聚类分析得到6组治疗银屑病的新处方。结论：通过中医传承辅助平台揭示《中华医典》治疗银屑病处方用药规律，治则上多从清热解毒，养血活血，祛风除湿等入手，挖掘出的新方可为后续临床银屑病内服药选择提供参考。

Analysis of the rule of prescription of Chinese Medical Code in the treatment of psoriasis based on traditional Chinese medicine inheritance support system

ABSTRACT Objective： To analyze the regularity of internal prescription of Chinese Medical Code in the treatment of psoriasis based on traditional Chinese medicine inheritance support platform V2.5 （TCMISP V2.5） software. Methods： The internal prescriptions for the treatment of psoriasis recorded in Chinese Medical Code were collected and their medication characteristics were analyzed. Results： Total 107 kinds of herbs were involved in 33 prescriptions and their medicinal properties are mostly bitter， sweet and pungent and belong to the liver， spleen and stomach meridian. The common herbs were mainly Fangfeng （Radix saposhnikoviae） ， Dahuang（Radix et rhizoma rhei）， Danggui （Radix angelicae sinensis）. There were 9 herb pairs with support degree ≥6， and 6 groups of new prescriptions for the treatment of psoriasis were obtained by unsupervised entropy cluster analysis. Conclusion： Through the TCMISP，the rules of prescriptions for the treatment of psoriasis are revealed in the Chinese Medical Code. The treatment is mostly from clearing away heat and detoxification， nourishing blood and promoting blood circulation， dispelling wind and removing dampness. The mined new prescription can provide reference for the follow-up selection of clinical oral medicine for psoriasis.

银屑病是一种常见的慢性免疫相关炎症性皮肤病，皮肤以红斑、丘疹、上面覆盖有鳞屑损害为主要表现，复发率较高，中医上称为“白H”“干癣”。现代医学认为其发病原因主要与免疫功能异常、遗传等因素有关[1]。本病临床上常用抗感染、免疫抑制剂及外用激素类药膏、维生素D3衍生物、维A酸及钙调磷酸酶抑制剂等治疗，但存在停药后易复发，尤其是不规范、长期应用强效激素会导致病情加重，甚至可能转化为红皮病型[2-3]。临床生物制剂如TNF-α拮抗剂、IL-12/IL-23拮抗剂、IL-17拮抗剂也起到重要作用，但其价格高昂，一定程度上限制了临床应用[4]。白H首见于清代《外科大成・卷四》，该书云：“白H，肤如疹疥，色白而痒、搔起白屑，俗呼蛇虱，由L邪客于皮肤，血燥不能容养所致。”[5]目前，中医药治疗银屑病已被证实具有独特优势。高东日等[6]应用紫草鳖甲四物汤治疗银屑病，证明该方疗效优于西医常规治疗组。崔利莎等[7]运用阳和汤加味治疗寻常型银屑病，发现其与单纯紫外线治疗相比可更有效地改善患者皮损面积及临床症状。
