糖尿病肾病患者25羟维生素D3与视网膜病变的相关性研究

  【摘要】 目的:探究糖尿病腎病25羟维生素D3[25-(OH)D3]与视网膜病变(diabetic retinopathy,DR)相关性。方法:选取本院2017年2月-2018年2月收治的140例糖尿病肾病患者,根据DR情况分为无DR组(n=50)、非增生期DR组(n=47)和增生期DR组(n=43),另选取同期来本院体检的46例健康者为对照组,采用电化学发光法检测所有受试者血清25-(OH)D3水平,探究糖尿病肾病25-(OH)D3与DR的相关性。结果:各组的高密度脂蛋白(high densitylipoprotein cholesterol,HDL-C)、血清总胆固醇(total cholesterol,TC)和低密度脂蛋白(lowdensity lipoprotein cholesterol,LDL-C)比较,差异均无统计学意义(P>0.05)。增生期DR组的25-(OH)D3低于无DR组、非增生期DR组和对照组,差异均有统计学意义(P<0.05),增生期DR组的糖化血清白蛋白(glycated albumin,GA)、血清糖化血红蛋白(glycosylated hemoglobin,HbA1c)均高于无DR组、非增生期DR组和对照组,差异均有统计学意义(P<0.05)。25-(OH)D3与DR之间呈负相关,为中相关性(r=-0.527,P<0.05),GA、HbA1c与DR之间呈正相关,为极弱相关性(r=0.107、0.216,P<0.05)。结论:25-(OH)D3影响着DR的出现及其病理过程,25-(OH)D3水平越低表明DR越重,为控制疾病,应积极检测25-(OH)D3水平,指导临床进行维生素D补充。

   【Abstract】 Objective:To explore the correlation of 25-hydroxy vitamin D3[25-(OH)D3]with diabetic retinopathy(DR)for diabetic nephropathy.Method:140 diabetic nephropathy patients treated from February 2017 to February 2018 in our hospital were selected and assigned to non-DR group(n=50),non-proliferative DR group(n=47)and proliferative DR group(n=43)according to the situation of DR.46 healthy check-ups were selected as control group.The electrochemiluminescence methods were used to detect 25-(OH)D3 contents.The correlation of 25-(OH)D3 with DR was explored.Result:The high-density lipoprotein cholesterol(HDL-C),total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)contents among each groups were not significantly different(P>0.05).25-(OH)D3 contents in the proliferative DR group was lower than those in the non-DR group,the non-proliferative DR group and control group,the differences were statistically significant(P<0.05).glycated albumin(GA)and glycosylated hemoglobin(HbA1c)contents in the proliferative DR group were higher than those in the non-DR group,the non-proliferative DR and the control group,the differences were statistically significant(P<0.05).25-(OH)D3 contents was negatively correlated with DR,and was moderately correlation(r=-0.527,P<0.05).GA and HbA1c contents were positively correlated with DR,and were extremely weakly correlations(r=0.107,0.216,P<0.05).Conclusion:The genesis and pathological courses of DR is affected by 25-(OH)D3.The lower 25-(OH)D3 contents indicate the aggravation of DR,in order to control the illness progression,it is necessary to monitor 25-(OH)D3 contents and supplement Vitamin D.
