盐酸氨溴索联合纤维支气管镜治疗重症呼吸机相关性肺炎的临床分析

  【摘要】 目的 观察盐酸氨溴索联合纤维支气管镜治疗重症呼吸机相关性肺炎(VAP)的效果。
  方法 120例重症VAP患者, 根据随机数字表法分为观察组和对照组, 各60例。对照组采用盐酸氨溴索治疗, 观察组在对照组基础上联合应用纤维支气管镜治疗。比较两组的临床疗效、机械通气时间、肺部感染控制时间及住院时间。结果 观察组治疗总有效率为96.7%(58/60), 明显高于对照组的80.0%(48/60), 差異具有统计学意义(P<0.05);观察组机械通气时间、肺部感染控制时间与住院时间分别为(6.49±1.31)、(6.82±1.48)、(16.71±2.21)d, 均明显短于对照组的(9.79±1.61)、(10.72±1.88)、(21.01±2.41)d, 差异均有统计学意义(P<0.05)。结论 临床上采用盐酸氨溴索联合纤维支气管镜治疗重症VAP, 可明显提升患者临床疗效, 缩短机械通气时间、肺部感染控制时间与住院时间, 值得临床推广。

  【Abstract】 Objective   To observe the effect of ambroxol hydrochloride combined with fiberoptic bronchoscopy in the treatment of severe ventilator-associated pneumonia (VAP). Methods   A total of 120 severe VAP patients were divided by random number table method into observation group and control group, with 60 cases in each group. The control group was treated with ambroxol hydrochloride, and the observation group was treated with fiberoptic bronchoscopy on the basis of the control group. Comparison was made on clinical efficacy, duration of mechanical ventilation, control time of pulmonary infection and hospitalization time between the two groups. Results   The total effective rate of treatment was 96.7%(58/60) in the observation group, which was obviously higher than 80.0%(48/60) in the control group, and the difference was statistically significant (P<0.05). The duration of mechanical ventilation, control time of pulmonary infection and hospitalization time were (6.49±1.31), (6.82±1.48) and (16.71±2.21) d respectively in the observation group, which was obviously shorter than (9.79±1.61), (10.72±1.88) and (21.01±2.41) d in the control group, and their difference was statistically significant (P<0.05). Conclusion   For the treatment of severe VAP, clinical application of ambroxol hydrochloride combined with fiberoptic bronchoscopy can significantly improve the clinical efficacy of patients, shorten the duration of mechanical ventilation, control time of pulmonary infection and hospitalization time. It is worthy of clinical promotion.
